Body Mass Index is the most widely used health screening tool in the world, as standardized by the World Health Organization. Your BMI is calculated by dozens of health apps, reported in every physical exam, and cited in public health guidelines across every developed country. It is also frequently misunderstood, misapplied, and dismissed — sometimes by the same people. Understanding what BMI actually measures, what the classification categories mean, where the tool genuinely falls short, and what to do with your result gives you a much more useful relationship with this number than either blindly following it or ignoring it entirely.
How BMI Is Calculated
BMI is calculated by dividing your weight in kilograms by the square of your height in meters: BMI = kg/m². In imperial units, the formula is BMI = (weight in pounds ÷ height in inches²) × 703. A person who is 5'9" (175 cm) and weighs 175 pounds (79.5 kg) has a BMI of 175 ÷ (69²) × 703 = 25.8.
The formula was developed by Belgian mathematician Adolphe Quetelet in the 1830s as a population-level statistical tool to study body proportions across large groups of people. It was never intended to be a diagnostic tool for individuals. It became widely used in clinical settings in the 1970s after physiologist Ancel Keys identified it as the most practical and widely applicable proxy for body fatness in large-scale epidemiological studies.
The Four BMI Categories
The World Health Organization and CDC classify adults (18 and older) into four categories:

What BMI Measures — And What It Doesn't
BMI measures weight relative to height. Nothing more. It cannot distinguish between fat mass, muscle mass, bone density, or water. This single limitation creates several well-documented categories of error:
- Muscular individuals: a competitive bodybuilder or professional athlete with 8% body fat and substantial muscle mass can register as 'overweight' or 'obese' using BMI, despite having excellent metabolic health markers.
- Older adults: as people age, they typically lose muscle mass while fat mass increases — a condition called sarcopenic obesity. An older adult can have a 'normal' BMI while carrying metabolically dangerous levels of visceral fat with significant muscle wasting.
- Ethnic differences: research shows that South Asian, East Asian, and other populations face elevated metabolic disease risk at lower BMI values than Western populations. The WHO has proposed lower cutoffs for certain Asian populations (23.0 for overweight, 27.5 for obesity).
- Women vs. men: women naturally carry more body fat than men at the same BMI. A 25 BMI in a woman and a 25 BMI in a man represent different body compositions.
Why Doctors Still Use BMI
Despite its limitations, BMI remains clinically useful because it is fast, free, requires no equipment, and correlates well with metabolic risk at a population level even if it misclassifies individuals. At the extremes — BMI below 17 or above 35 — classification errors are rare. Most of the miscategorization occurs near the category boundaries (approximately BMI 25 and 30) in muscular individuals.
The American Medical Association updated its policy in 2023 to caution against using BMI as a sole diagnostic measure, recommending it be used alongside waist circumference, body fat percentage, and metabolic markers (blood pressure, blood glucose, lipid panel). This represents a meaningful shift in how BMI is used clinically — from a standalone diagnostic to one data point among several.
What to Do at Each BMI Range
If your BMI is in the underweight range (below 18.5): consult a healthcare provider. Unintentional weight loss or inability to maintain weight can indicate medical conditions including thyroid disorders, gastrointestinal disease, or eating disorders. Nutritional deficiencies at this range affect bone density and immune function.
If your BMI is in the normal range (18.5–24.9): maintain current habits with attention to waist circumference. A normal BMI with a high waist circumference (above 35 inches for women, 40 inches for men) still indicates elevated cardiovascular risk.
If your BMI is in the overweight range (25–29.9): assess alongside body composition, waist measurement, and metabolic markers. For many people in this range, waist circumference and physical fitness level are more predictive of health outcomes than BMI alone.
If your BMI is 30 or above: discuss with a healthcare provider. The cardiovascular, metabolic, and joint health benefits of reducing BMI toward the normal range are well documented, but sustainable interventions vary by individual.
Better Measures to Use Alongside BMI
Given BMI's limitations, clinicians increasingly recommend pairing it with waist circumference and, where accessible, body fat percentage. Waist circumference is a better proxy for visceral fat — the metabolically active fat that accumulates around internal organs and drives cardiovascular and metabolic disease risk independently of total body weight. The risk thresholds widely used in clinical practice are 35 inches (88 cm) for women and 40 inches (102 cm) for men. Exceeding these cutoffs at any BMI level indicates elevated cardiometabolic risk.
Body fat percentage, measured via DEXA scan, hydrostatic weighing, or validated bioelectrical impedance devices, provides the most direct picture of body composition. Healthy body fat ranges are generally defined as 21–33% for women and 8–25% for men, though these vary somewhat by source. Unlike BMI, body fat percentage can correctly identify a lean, muscular individual as healthy and flag an individual with a 'normal' BMI who has excess fat relative to muscle as metabolically at risk — the condition sometimes called 'normal weight obesity.'
